Do NOT send any personal information to email like the one above!

This looks like an attempt to steal someone's ID. With the information they are asking for, they could steal someone's id and really trash someone's credit rating and leave them with more hell than they thought humanly possible. My advice: delete it and save yourself from the nightmare.
